TESTING ACCOMMODATIONS: COLLEGE BOARD AND ACT
HOW TO APPLY FOR COLLEGE BOARD TESTING ACCOMMODATIONS:
Dear Student and Parent/Guardian:
If wish to apply for testing accommodation(s) on College Board tests (SAT, PSAT/NMSQT, and/or Advanced Placement Exams) due to disability, please click on the following link for a copy of “CONSENT FORM FOR ACCOMMODATIONS REQUEST”.
Complete the form and send it to the District Office for Special Services, Services for Students with Disabilities Coordinator. The signed “SSD CONSENT FORM” will authorize the District: to release to the College Board copies of your records that document the existence of a disability and need for testing accommodations; to release any other information in the school's custody that the College Board requests for the purpose of determining my eligibility for testing accommodations on College Board tests; and to discuss my disability and accommodation needs with the College Board.
By submitting a signed SSD Consent Form, you also grant the College Board permission to receive and review your records, and to discuss your disability and needs with school personnel and other professionals.

How to Request ACT Testing Accommodations
If you are a student or parent/guardian requesting testing accommodations for the ACT, please follow the steps below. It is important to begin this process as early as possible because ACT must review and approve accommodation requests before the test date.
Step 1: Register for the ACT
Create or log in to your MyACT account and register for the ACT test date you plan to take.
During the registration process:
- Select “Yes” when asked if you need testing accommodations.
- Enter Connetquot High School as your high school.
- Use the Connetquot High School ACT High School Code: 330449.
Important: You must indicate that you need accommodations when you register for the ACT.
Step 2: Contact Connetquot High School
After registering for the ACT, contact the person responsible for submitting ACT accommodation requests at Connetquot High School.

You may also speak with your school counselor or special education case manager, who can help direct you to the appropriate person.
Step 3: Provide Your Documentation
Your school will need documentation that supports your request for ACT testing accommodations.
Depending on your circumstances, documentation may include:
- An IEP (Individualized Education Program)
- A 504 Plan
- A psychological or educational evaluation
- Medical documentation
- Other professional documentation supporting the need for accommodations
Please provide your documentation to the school as soon as possible.
Step 4: School Submits Your Request to ACT
The school accommodation coordinator will review your documentation and submit your request through ACT's Test Accessibility and Accommodations (TAA) system.
Students and parents/guardians do not submit the formal accommodation request directly to ACT. The request must be submitted by an authorized school official.
Step 5: Wait for ACT's Decision
ACT will review the accommodation request and supporting documentation.
ACT generally reviews complete requests within 5–10 business days. Your school official will receive notification of ACT's decision.
Once a decision is received, the school will communicate the approved accommodations to the student and/or parent/guardian as appropriate.
Important Information
Having an IEP or 504 Plan does not automatically mean that all of the accommodations a student receives at school will be approved for the ACT.
ACT makes the final determination regarding accommodations based on its requirements and the documentation submitted by the school.
Start Early!
Do not wait until shortly before the ACT test date to begin the accommodation process. Register for the ACT, indicate that you need accommodations, and contact the school as soon as possible.
