SENIOR TRIP – MANDATORY STUDENT/PARENT/GUARDIAN MEETING ON WEDNESDAY, OCTOBER 8, 2025 AT 6:30 PM.  CODE OF CONDUCT WILL BE REVIEWED, SIGNED AND COLLECTED WITH 1ST PAYMENT COLLECTION.

  • Time: 6:30 PM
  • Location: CHS Auditorium

There will be a mandatory meeting for Parent(s)/Guardian(s) and Students who plan to attend the Senior Trip to Woodloch Pines from Sunday, January 11, 2026 through Tuesday, January 13, 2026.

  • Please print the packet SINGLE SIDED
    • Complete ALL necessary sections on the forms
    • CODE OF CONDUCT will be completed at the mandatory meeting
    • FORM – PROVIDER ATTESTATION AND PARENT PERMISSIONS FOR INDEPENDENT MEDICATION CARRY AND USE
      • The form above must be completed by a Health Care Provider and parent/guardian for student to self administer the following medications for:
        • Allergy and requires Epinephrine Auto-injector
        • Asthma or respiratory condition that requires an inhaled respiratory medication
        • Diabetes and requires Insulin/Glucagon/Diabetes Supplies

Parking at Connetquot High School:

Parking on the Connetquot High School campus is a SENIOR PRIVILEGE and you must have a CHS Parking Tag.  This tag is available through the CHS Security Office after completing a parking application.  

 

  • SENIORS WHO ARE REPORTED DRIVING RECKLESSLY IN THE LOCAL COMMUNITY WILL LOSE THE PRIVILEGE TO PARK IN THE SENIOR PARKING LOT.

 

Only SENIORS with a New York State Class D driver’s license can apply for a parking tag.  Please note, a Class D License is issued only to people 18 years of age or older or to a person who is 17 years of age and has successfully completed an approved Driver Education course and received your certificate of completion.  By law, you cannot drive to school with a Junior License (Class DJ).  

 

  • THERE IS NO COST OR FEE TO submit a Senior Parking Application, but you must have a NYS Class D Driver's License to qualify for a parking permit.

 

  • ALL seniors who submit a parking application and receive a parking tag will be assigned a specific parking space number.

 

 

PARKING TAGS MUST BE DISPLAYED FROM YOUR REARVIEW MIRROR WHEN YOU PARK ON SCHOOL GROUNDS.  Violators will receive a warning.  Ongoing issues may result in loss of parking privileges.

 

If you are identified parking on school grounds without a valid parking tag you can be prohibited from parking on school grounds in the future.

 

  • JUNIORS ARE NOT PERMITTED TO PARK ON SCHOOL GROUNDS

Class of 2026 -  How to received your Senior Parking Tag?

  • Even if you paid to paint a Senior Parking Space you must apply for a Senior Parking Tag.
  • During the last two weeks of August the high school will begin accepting Class of 2026 Senior Parking Permit Applications.

Submitting Senior Parking Applications will take place during the school year during periods 1 through 5 at the Security Desk located by the back door to the student parking lot.


IMPORTANT INFORMATION FOR A STUDENT WITH A NYS JUNIOR DRIVERS LICENSE (CLASS DJ):

  • Per the NYS Department of Motor Vehicles - Any person with a NYS Class DJ drivers license cannot drive to or from the high school without a parent or guardian in the vehicle.
